(MDs and DOs) & PAsThis activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the joint sponsorship of the American Medical Directors Association (AMDA) and the Florida Medical Directors Association. The American Medical Directors Association is accredited by the ACCME to provide continuing medical education for physicians.
Credit Designation Statement: The American Medical Directors Association designates this educational activity for a maximum of 17.0 category 1 credit(s) toward the AMA Physician’s Recognition Award. Each physician should claim only those credits that he/she actually spent in the activity.
CMD Credit Statement: This seminar has been approved for up to 17.0 credit hours in medical direction toward certification as a Certified Medical Director in Long-Term Care (CMD). The AMDA CMD program is administered by the American Medical Directors Certification Program. Each physician should claim only those hours of credit actually spent on the activity.
This program has been approved by the American Osteopathic Association for 17.0 hours of AOA Category 2-A credit.
Note: Physicians may earn an additional 1.25 credit hours (provided by a third party), by attending an industry sponsored-symposium which starts 10:45 a.m., Friday, Oct. 17, 2003.
Contact Hours for Nurse Practitioners
This activity has been approved by the Alabama State Nurses Association (ASNA), for up to 21.0 contact hours of continuing education (Activity #2-0.881). ASNA is accredited as an approver of continuing education in nursing by the American Nurses Credentialing Center’s Commission on Accreditation.
Contact Hours for Nurse Administrators
This educational program has been approved for up to 21.0 contact hours by the Florida Association Directors of Nursing Administration/LTC (Florida Board of Nursing Provider Number FBN3192).
Pharmaceutical Continuing EducationThe pharmacist program is co-sponsored by the Florida Chapter of the American Society of Consultant Pharmacists and the Florida Medical Directors Association. Participants may only apply for up to 7.0 hours of general pharmaceutical continuing education in addition to 12.0 hours of consultant recertification credit for Florida consultant pharmacist license. Duplication of credits is not permissible. You must submit a completed course evaluation in order to achieve credit. COJM International Services Inc. is accredited by the American Council on Pharmaceutical Education as a provider of continuing pharmaceutical education.
Consultant Recertification: This program is approved by the Florida Board of Pharmacy for 12.0 hours of Florida Consultant Pharmacist Recertification credit. To receive credit, participants must complete 12.0 hours of credit hours during this CE program. Please allow 4-6 weeks for processing of your CE certificate of attendance.
This educational offering has been submitted to the National Continuing Education Review Service (NCERS) of the National Association of Boards of Examiners for Nursing Home Administrators (NAB) for up to 17.0 clock hours. The administrators program is co-sponsored by the Florida Chapter of the American College of Health Care Administrators and the Florida Medical Directors Association.
